INFORMACIÓN QUE NECESITAMOS DE USTED ANTES DEL CAMPAMENTO
Por favor, ayúdenos a hacer del campamento la mejor experiencia posible completando este FORMULARIO.
Los elementos más importantes del formulario son:

Tamaños de camisetas (puede responder solo a esa pregunta, enviar el formulario y luego enviar otro formulario con el resto de las respuestas más tarde).

Registros de vacunación (gracias a quienes ya los han proporcionado) - deben enviarse antes de las 5 pm, lunes 24 de junio.

IMPORTANTE:
El estado de NJ requiere registros de vacunación para todos los jóvenes que asisten a campamentos en el estado – por favor, use este ENLACE para enviar su estado de vacunación. También puede usar este enlace para declarar que ha optado por no vacunarse.
