The State of New Jersey requires us to collect immunization information for each camper:
 

Please email the following information about your child by 5pm, Monday, June 24 (YW leaders and Bishops / Branch Presidents - please follow up with parents to make sure this is done):

1. Most recent immunization records (e.g. the last immunization records you gave your child's school or received from your child's physician) showing the vaccinations required for school attendance; and/or

2. A statement from a physician that immunization is in progress; and/or

3. A statement signed by you requesting a medical exemption from immunization; or

4. A statement signed by you requesting a religious exemption from immunization.

The vaccinations required for school attendance (6th grade and higher) are as follows:
